Thank you for choosing Berkshire Speech + Language. By engaging our services you expressly agree to all of the following conditions and agree to be bound by them.
Berkshire Speech + Language renders services regardless of race, color, creed, social status, national origin, sexual orientation, handicap, or gender.
Responsibility for Payment
Berkshire Speech + Language does not participate in any insurance plan and clients are responsible for all payments.
At the time a new client is registered through our online Client Portal, credit card information is collected and stored securely. The credit card will be automatically charged for all services rendered on the day the services are provided. This will streamline and simplify all client transactions and allow us to focus on providing the best services possible for you.
If, for any reason, you do not wish to provide your credit card information to us so that charges can be made in the manner described above, payment in full for all services must be provided by BerkShares, cash, or check prior to the beginning of each session and a 15% surcharge will be added to the session fee. If prior payment is not made, the session will not begin.
Your credit card information will be stored securely and will not be shared with any other person or organization.
At your request Berkshire Speech + Language will furnish you with a detailed statement or superbill with diagnosis and procedure codes for submission to your insurance plan. However, you are responsible for all payments to BS+L as outlined above whether or not your insurance plan covers our services in whole or part. BS+L makes no representation whatsoever that any services will be covered by any insurance plan in whole or in part.
You understand and agree that for sessions cancelled with less than 24 hours notice (unless due to sickness, family emergency, or inclement weather), a cancellation fee of $50.00 will be charged. You also understand and agree that if you do not show for a scheduled session, you will be charged the full amount of a session, depending upon the type of scheduled service. Cancellation and No Show fees will be due prior to scheduling any further appointments. This applies to all clients without exception.
You understand that sessions cancelled with more than 24 hours notice will not be charged a cancellation fee. However, we encourage scheduling make-up sessions for these and all other sessions in order to ensure optimal therapeutic progress.
Repeated cancellations may result in the loss of your regularly scheduled time slot or termination of service.
An appointment cancelled with less than 24 hours notice due to inclement weather will not incur a cancellation fee.
You understand that once your weekly treatment appointment schedule has been determined, BS+L is often unable to accommodate changes on an ad-hoc or temporary basis. When a permanent change in time is needed, you agree to give as much advance notice as possible for BS+L to attempt to accommodate your request. When BS+L must initiate a permanent change of schedule, the therapist will give you at least a one week's notice and try to accommodate your scheduling needs, but BS+L makes no guarantee to that effect.
You understand and agree that in order to receive maximum benefit from treatment, it is important for treatment to occur routinely. You understand that notification of vacations or family obligations is requested at least two weeks prior to the expected absence to facilitate rescheduling appointments.
Acknowledgment of Risk
You acknowledge that, while minimal, there is some risk inherent in the use of some therapy equipment. You agree to indemnify and hold BS+L harmless from any and all losses and claims for any injuries or other damages occurring to your child or yourself or your belongings from the use of therapeutic equipment at your home, or any other location where we render services.
